The forms listed below are provided in advance of an interview should you want to move forward promptly: the Interview Sheet and a Domestic Relations Financial Affidavit for both Fulton and Non-Fulton County residents. They may either be downloaded as PDF to fill out by hand or as an Text or RTF file, which any word processor can open and edit.

This form is specifically for the initial interview. It is confidential, and will be treated in that fashion. Please spend some time preparing the form but as it is only the beginning of our fact finding, you do not have to be as detailed as the form calls for.

This form includes General Information, Information Concerning Present Marriage, Work Experience, Assets, Pre-Marital Assets, Automobiles, Bank Accounts, Debts, Insurance, Counseling, Health, Causes of Separation and Custody.
